​At Northern Colorado Medical (“NCM,” “we,” “us”), your privacy matters. This Privacy Policy explains what we collect through our website only and how we use it. It does not cover your medical records or protected health information (PHI). For PHI, please request our separate HIPAA Notice of Privacy Practices (NPP).

Do not send PHI via the website or email

​Please do not include medical details, images, or other PHI in website forms or email. When PHI is required, we will provide a secure option (e.g., patient portal, encrypted upload, or in-office forms).

Information we collect

You provide: your name, email, phone, and message content when you contact us; optional business details if you inquire about billing/vendor matters; your mobile number if you opt in to texts.
Automatic data: typical server logs (IP address, device/browser type, pages viewed, timestamps, referring URLs).
 

Analytics (Google Analytics 4)

We use GA4 to understand site usage and improve the site. GA4 uses cookies and similar technologies to collect pseudonymous information such as pages viewed, approximate location (city/region), device/browser type, and time on page. We configure GA4 so that:

• We do not send PHI or medical details to Google.

• GA4 does not store IP addresses (it derives approximate location and discards IP).

• Google Signals / Ads Personalization are disabled, and we do not use GA4 for targeted advertising or link it to Google Ads.

• Event data retention is set to 14 months or less and used only to operate and improve the website.
  We do not place GA on pages that collect medical information or on any patient portal.

 

How we use information

• Operate, maintain, and improve the website

• Respond to inquiries and provide requested information

• Send administrative notices; send marketing messages only with your consent (you can opt out any time)

• Protect the site from fraud, abuse, and security risks

• Comply with law, subpoenas, court orders, or legal process

 

Text messages (SMS)

If you opt in, you agree to receive texts from NCM. Message & data rates may apply. Frequency varies. Reply STOP to end, HELP for help. Opting out of texts does not affect non-marketing communications we’re legally allowed to send.

 

Third parties / service providers

We may use trusted service providers (e.g., website host, email service, analytics) who process data under our instructions and may not use it for their own purposes. We do not sell personal information or share it for cross-context behavioral advertising.

 

Data retention

We keep website data only as long as needed for the purposes above, to comply with law, or to resolve disputes, then delete or de-identify it.

 

Security

We use administrative, technical, and physical safeguards appropriate for a small medical practice website. No system is perfectly secure; please don’t send sensitive information through unsecured channels.

 

Children

This site isn’t directed to children under 13, and we don’t knowingly collect personal information from them.

Your choices & Colorado privacy rights

• Opt out of marketing emails at any time via the unsubscribe link.

• Opt out of SMS at any time by replying STOP.
